Thank you for your interest in my
research. My name is Adrian Willey; I am a doctoral
candidate at the California School of Professional Psychology at
It will take an estimated 45 minutes to complete the research, which
involves filling out a number of questionnaires as well as a background
information form.
This survey is designed to be completely anonymous. While this study is designed to protect
your anonymity, some of your responses will be shared with the authors of the
questionnaires to aid them in improving their measures. No personal information will be shared with
anyone nor will any personal information be linked to your
responses.
Participation is voluntary. You are not required to take part
in this study. You may exit the study at any point without
consequences.
Some of the questions in this research may stimulate your
thinking about your past and your current relationship, as well as other events
which could be helpful to you and may promote insight into important
aspects of your life. Because
some questions touch on sensitive issues including questions about stressful
events which may have occurred in the past, there is the possibility that they
may arouse distress. If you
experience any distress due to your participation I can refer you to a licensed
therapist for a one time consultation at no cost to you.
Your participation is appreciated. As a token
of my appreciation, I will be offering a lottery for 7 cash “prizes” of $100
each, given to randomly chosen participants. Participation in the lottery will
require you to provide contact information via email or voicemail to an address
or phone number provided at the end of the survey. The contact information you provide will
not be linked in any way to the information you provide as part of the
survey. It will not be possible for
ANYONE to identify what research response belongs to which lottery entry. The lottery will be held upon the
conclusion of this study, approximately December
2009.
If you would like to receive the results of this study,
please contact me via the email address or phone number listed below. The results will be available at
the conclusion of my work on this research and after the summary has been
approved by the Alliant International University Institutional Review
Board. In order to receive a copy
of the results of this study you will have to provide your postal or e
mail address. While this would
involve giving personally identifiable information, I will not be able to
connect this information with any individual's results. Every effort will be made to protect
your confidentiality.
